What is the Form 150?

The Special Events Proceeds Form 150 (aka Form 150) is used to record funds collected at special events. To obtain a copy of the Form 150, please click the following link: Special Events Proceeds Form 150.

The form may be completed on a computer and printed before submission. Place the page labeled 'Audit copy' with any event proceeds in the Form 100 Pledge Form Carrier. Retain the 'Agency Copy' for records.

A sample of a Form 150 is included below. Note that this sample form is completed with "Combined Federal Campaign" in place of a valid agency and bureau name, and the reporting unit number is an example. Be sure to complete the Form 150 with the correct information for department/agency, bureau/office, and reporting unit number. Look up agency information with the Agency Search tool.
